कैसे होता है भेंगेपन का इलाज़ ? भेंगेपन में उपचार का लक्ष्य होता है :

भेंगापन क्या है ?


भेंगापन या स्ट्राबिस्मस  (तिरछी आँख )में आँखें एक सीध में नहीं होती ,इस वजह से अलग दिशा में होती हैं। एक आँख इच्छित दिशा  की ओर होती है तो दूसरी किसी भी ओर हो सकती है। 

भेंगापन स्थाई भी हो सकता है या कुछ समय बाद अपने आप ठीक और दोबारा भी हो सकता है। यह वयस्कों की अपेक्षा बच्चों में ज्यादा पाया जाता है।

आँखें एक सीध व संतुलन में कैसे बनी रहतीं हैं ?

आँखों को एक सीध एवं संतुलन में बनाये रखने का काम आँख के दोनों ओर स्थित छ: मांसपेशियाँ करतीं हैं ,जो आँख को सभी इच्छित दिशाओं में घुमाती हैं। इन मांसपेशियों को मस्तिष्क नियंत्रित करता है ,यही कारण है कि दोनों आँखें एक समय पर एक ही इच्छित दिशा की ओर होती हैं। दोनों आँखों से अलग अलग ली गई तस्वीर को मस्तिष्क एक कर हमें उस छवि से अवगत कराता है। 

भेंगापन कैसे होता है?

भेंगेपन का कोई निश्चिय ज्ञात कारण  नहीं है। यह एक सामान्य आँख में भी हो सकता है और मोतियाबिंद ,चोट ,कार्निया में धुंधलेपन या रेटिना सम्बन्धी दोष से प्रभावित आँख में भी हो सकता है। मस्तिष्क में किसी प्रकार के विकार जैसे दिमागी फ़ालिज़ ,डाउन सिंड्रोम ,ब्रेन ट्यूमर आदि के कारण  भी भेंगापन हो सकता है। 

क्या भेंगेपन से दृष्टि प्रभावित हो सकती है ?

बेशक ,इससे दृष्टि प्रभावित हो सकती है। यदि भेंगापन है तो तिरछी आँख का प्राय : प्रयोग कम होता है ओर मस्तिष्क इससे प्राप्त होने वाली छवि की उपेक्षा करने लगता है। इस कारण एम्बिलियोपिया (अलसाई आँख )हो जाता है, जिससे दृष्टि कमज़ोर हो जाती है। भेंगेपन  से पीड़ित आधे से अधिक बच्चों के साथ ऐसा ही होता है। इसके अलावा स्थाई या लम्बे समय से भेंगेपन के कारण दूर की चीज़ को गहराई से देखने की क्षमता (BINOCULARITY )ओर किसी चीज़ को गहराई से देखने की क्षमता (stereopsis )कम हो जाती है ,इन दोनों की आवश्यकता महीन काम करने में ,गाड़ी  चलाने व क्रिकेट जैसे खेलों में होती है। 

भेंगेपन की पहचान कैसे करें ?

बच्चों में भेंगेपन के अधिकाँश मामलों में माता पिता खुद ही इसकी पहचान कर लेते हैं ,जिन बच्चों में आँखें तीन माह की उम्र के बाद भी तिरछी हों तो भेंगेपन को लेकर उस पर गौर करना चाहिए ,वहीँ कुछ बच्चों में सिर की असामान्य बनावट के कारण भी इसका आभास होता है। हो सकता है कि बड़े बच्चों में चपटी नाक एवं आँख -नाक के बगल में मुड़ी त्वचा से ,उनमें भेंगापन न होते हुए भी होने का एहसास (pseudosquint)हो सकता है। 

भेंगेपन के इलाज़ के लिए डॉक्टर  से कब संपर्क करें ?

माता पिता को सलाह दी जाती है कि उनके बच्चे में जब भी भेंगेपन के लक्षण दिखें ,जल्दी से अच्छे नेत्र विशेषज्ञ से संपर्क करें। अलसाई आँख की मौज़ूदगी पर भी ध्यान देने की विशेष ज़रुरत होती है। चिकित्सीय मदद से आभासी भेंगापन (फाल्स स्क्विंट ) भी दूर हो जाता है। 

अलसाई आँख (एम्बलियोपिया )का उपचार क्या है ?

अलसाई आँख (एम्बलियोपिया )या दृष्टि निर्बलता का उपचार भेंगापन की सर्जरी से पहले किया जाता है। आँखों की विस्तृत जांच के बाद बच्चे को ज़रुरत होने पर चश्मा लगाने की सलाह दी जाती है ,फिर माता पिता को बच्चे की स्वस्थ  आँख पर रोज़ाना कुछ निश्चित समय के लिए पट्टी बाँधने को कहा जाता है। यह एक प्रकार का व्यायाम है और अलसाई आँख को जागृत करता है। 

अलसाई आँख का उपचार कब शुरू करें ?

स्वस्थ आँख पर पैच लगाना या पट्टी बाँधना जितनी जल्दी हो सके शुरू कर देना चाहिए। आँख पर पट्टी या पैच बाँधने को लेकर अभिभावक बच्चों के साथ कोई ढिलाई नहीं बरतें ,बच्चे की उम्र जितनी कम होगी आँख पर पैच या पट्टी बाँधने का परिणाम बेहतर होगा। इसके अलावा दृष्टि आंकलन के लिए नियमित नेत्र जांच बेहद ज़रूरी है। 

कैसे होता है भेंगेपन का इलाज़ ?

भेंगेपन  में उपचार का लक्ष्य होता है :


(१) दृष्टि की हिफाज़त 

(२) दोनों आँखों को एक सीध में लाना 

(३) आँखों की बाइनाकुलर विजन में सुधार 

आँख पर पैच लगाने का दौर सफलतापूर्वक पूरा होने के बाद भेंगेपन के इलाज़ के बारे में विचार किया जाता है। इलाज़ में सर्जरी भी शामिल होती है। 

क्या सभी बच्चों के मामले में सर्जरी की ज़रुरत होती है ?
नहीं। बच्चों में भेंगापन के कुछ मामले जिसे एकोमोडेटिव स्क्विंट कहते हैं ,उनमें केवल चश्मा लगाने से सुधार आ जाता है। हालाकि बाकी प्रकार के भेंगेपन के उपचार के लिए सर्जरी की ज़रुरत होती है। 

अभिभावकों के लिए मुख्या बातें :

(१) बच्चों के  जन्म के चार माह बाद भी भेंगेपन की स्थिति बनी रहती है तो नेत्ररोग विशेषज्ञ को ज़रूर दिखाएँ 

(२) बच्चों में होने वाले भेंगेपन  के कुछ मामलों में केवल चश्मे की मदद से उपचार सम्भव है। 

(३) यदि ज़रुरत हो तो सर्जरी छह :माह की उम्र में करा सकते हैं उपचार में देरी अलसाई आँख का कारण बन सकता है। 

(४) जल्द उपचार से बच्चे की दृष्टि बेहतर होती है 


Strabismus


What Is Strabismus?

Strabismus is a visual problem in which the eyes are not aligned properly and point in different directions. One eye may look straight ahead, while the other eye turns inward, outward, upward, or downward. The eye turn may be consistent, or it may come and go. Which eye is straight (and which is misaligned) may switch or alternate.

Strabismus is a common condition among children. About 4 percent of all children in the United States have strabismus. It can also occur later in life. It may run in families; however, many people with strabismus have no relatives with the problem.

Infantile esotropia, where the eye turns inward, is the most common type of strabismus in infants. Young children with esotropia cannot use their eyes together.Accommodative esotropia is a common form of esotropia that occurs in children usually 2 years or older. In this type of strabismus, when the child focuses the eyes to see clearly, the eyes turn inward. This crossing may occur when focusing at a distance, up close or both.

Exotropia, or an outward-turning eye, is another common type of strabismus. This occurs most often when a child is focusing on distant objects. The exotropia may occur only from time to time, particularly when a child is daydreaming, ill or tired. Parents often notice that the child squints one eye in bright sunlight.

Strabismus is when both eyes do not look at the same point at the same time because the eye muscles do not work together well to control the eye movement. Often time’s strabismus begins in early childhood and appears as though the child is “cross-eyed.”

Children, who complain about their eyes being tired, blurred or double vision or light sensitivity should be evaluated by an optometrist or an ophthalmologist for possible strabismus.
A child will not outgrow this condition and if left uncorrected, strabismus can cause permanent in the form of a lazy eye.
Although childhood strabismus does not have any known cause, it tends to run in the family. Adults may develop strabismus due to eye or blood vessel damage, an eye or brain tumor, Graves’ disease, stroke or various muscles or nerve disorders.

Strabismus Causes

Six eye muscles, controlling eye movement, are attached to the outside of each eye. In each eye, one muscle moves in the eye to the right, and one muscle moves the eye to the left. The other four muscles move it up or down and at an angle.

To line up and focus both eyes on a single target, all of the muscles in each eye must be balanced and working together. In order for the eyes to move together, the muscles in both eyes must be coordinated. The brain controls these muscles.
With normal vision, both eyes aim at the same spot. The brain then combines the two pictures into a single, three-dimensional image. This three-dimensional image gives us depth perception.
When one eye is out of alignment, two different pictures are sent to the brain. In a young child, the brain learns to ignore the image of the misaligned eye and sees only the image from the straight or better-seeing eye. The child then loses depth perception.

Adults who develop strabismus often have double vision because their brains have already learned to receive images from both eyes and cannot ignore the image from the turned eye. A child generally does not see double.
Strabismus is especially common among children with disorders that may affect the brain, such as:
  • Cerebral palsy;
  • Down syndrome;
  • Hydrocephalus;
  • Brain tumors;
  • Prematurity.
A cataract or eye injury that affects vision can also cause strabismus. The vast majority of children with strabismus, however, have none of these problems. Many do have a family history of strabismus.

Strabismic amblyopia

Good vision develops during childhood when both eyes have normal alignment. Strabismus may cause reduced vision, or amblyopia, in the misaligned eye.
The brain will pay attention to the image of the straight eye and ignore the image of the crossed eye. If the same eye is consistently ignored during early childhood, this misaligned eye may fail to develop good vision, or may even lose vision. Strabismic amblyopia occurs in approximately half of the children who have strabismus.
Patching




Treatment


Amblyopia can be treated by patching or blurring the stronger eye to strengthen and improve vision in the weaker eye. If amblyopia is detected in the first few years of life, treatment is usually successful. If treatment is delayed, amblyopia may become permanent. As a rule, the earlier amblyopia is treated, the better the result for vision.

Glasses, patching, medicines and surgery are the most common treatments for strabismus. Glasses can correct strabismus if the eyes alignment is just slightly off. Sometimes, eye exercises may be helpful.
In most severe cases, surgery is the only alternative. The doctor changes the length or position of the muscles around the eye to help it align better.

Strabismus Symptoms
The main sign of strabismus is an eye that is not straight. Sometimes children will squint one eye in bright sunlight or tilt their head to use their eyes together.


PseudostrabismusThe eyes of infants often appear to be crossed, though actually they are not. This condition is called pseudostrabismus. Young children often have a wide, flat nose and a fold of skin at the inner eyelid that can make eyes appear crossed. This appearance of pseudostrabismus may improve as the child grows. A child will not outgrow true strabismus.
An ophthalmologist can distinguish true strabismus and pseudostrabismus.
